GRADUATE MEDICAL EDUCATION POLICIES AND PROCEDURES

Subject: Monitoring Fatigue in Residents

Policy:
The Department of Pathology and Cell Biology of the New York Presbyterian Hospital, Columbia University Medical Center, in compliance with the New York State Department of Health Regulations and ACGME program requirements, requires that all post-graduate trainees be monitored for signs of fatigue.

It is the responsibility of the Program Director / designee to monitor the on-call and assignment schedules of all post graduate trainees to ensure that residents are receiving ample time for rest, in compliance with NYS and ACGME duty hour regulations.

The program must educate faculty and residents about the signs and symptoms of fatigue. The program shall determine the best way to accomplish this and may also access a list of educational resources on the NYPH GME web-site

Any post-graduate trainee who is experiencing fatigue should immediately notify his/her program of his/her state of fatigue. When a program director / designee determines that a post-graduate trainee is too fatigued to perform his/her duties effectively, the program director will arrange for the trainee to be temporarily relieved from all on-call and clinical duties.

It is the responsibility of the Program Director / designee to notify the appropriate individuals of the change in the on-call and clinical assignment schedule.

Fatigue in physician trainees may compromise patient safety and the well-being of the trainees and limit the educational opportunities provided by training program. Therefore, the Program Director monitors the assignment schedules of fellows to ensure that fellows are receiving ample time for rest, in compliance with NYS and ACGME duty hour regulations.

The trainees receive education regarding recognition of mental stress and fatigue during the required hospital-wide orientation prior to the start of their training.

In addition, the program director educates faculty, fellows, and trainees about the signs and symptoms of fatigue. When the program director determines that a fellow is too fatigued to perform his/her duties effectively, the program director will arrange for the trainee to be temporarily relieved from all clinical duties.
